Spa Consent Policy
Effective Date: April 11, 2026
Centered SPA LLC ("Centered"), located at 117 W Allen St, Wentzville, MO 63385, requires informed consent before providing spa, beauty, and wellness treatments. By scheduling and receiving services at our facility, you acknowledge and agree to the following.
1. General Consent for Treatment
By receiving spa services, you voluntarily consent to the treatments and procedures discussed with your service provider. You understand that spa treatments, while generally safe, carry inherent risks including but not limited to allergic reactions, skin irritation, bruising, soreness, and temporary discomfort.
2. Health Disclosure
You are responsible for informing your service provider of any:
- Known allergies or skin sensitivities
- Current medications (including topical treatments)
- Pregnancy or nursing status
- Recent surgeries or medical procedures
- Chronic health conditions (e.g., diabetes, heart conditions, skin conditions)
- Areas of concern or discomfort on your body
3. Treatment Expectations
- Results vary per individual and are not guaranteed
- Some treatments may require multiple sessions for optimal results
- Post-treatment care instructions should be followed as directed
- You may experience temporary redness, sensitivity, or mild discomfort after certain treatments
4. Right to Refuse or Stop Treatment
You have the right to refuse any treatment or ask your service provider to stop at any time during a session. Please communicate any discomfort immediately to your provider.
5. Photography & Recording
Photography, video recording, or audio recording is not permitted in treatment rooms or private areas of our facility without prior written consent from Centered SPA LLC.
6. Personal Belongings
Centered SPA LLC is not responsible for lost, stolen, or damaged personal belongings. We recommend leaving valuables at home or in your vehicle.
7. Age Requirement
Clients under the age of 18 must have a parent or legal guardian present and provide written consent before receiving any services.
